The My Kids Hollywood 2009 SuperStar™ Search begins September 1, 2008 and ends August 1, 2009.

The Contest Sponsor is My Kids Hollywood, LLC., hereinafter referred as “MKH”, 
3225 McLeod Drive, Suite 100, Las Vegas, NV 89121.

Eligibility
Contest is open to Parents or Legal Guardians, age eighteen (18) or older at the time of entry, of children from birth to seventeen (17) years of age. Current Kids Hollywood Connection clients, employees of MKH and its parents, affiliates, subsidiaries, agents, advertising and promotional agencies and their immediate family or members of the same household are not eligible. 

CONTEST IS VOID WHERE PROHIBITED

The entrant assumes full responsibility for knowing if their local, state or federal laws prohibit photo contests. MKH in no way accepts responsibility for determining your legal eligibility as it pertains to your local, state or federal laws. 

By submitting an entry, an entrant agrees to allow MKH to use and publish submitted photo(s), his or her name, child’s name, age and city to post on the MKH web site and/or marketing/promotional materials, without compensation unless prohibited by law. All entries and all rights of ownership in and to the entries, including all rights to use, reproduce, publish, modify, edit, and distribute the same will become the exclusive property of MKH and will not be returned. MKH reserves the right to edit, modify, adapt, copyright, publish, use, and reproduce any and all entries without further compensation. MKH, and its parents, affiliates, subsidiaries, agents, advertising and promotional agencies, are not responsible for lost, late, misdirected, or incomplete entries. 

How to Enter

• For the easiest way to enter, visit www.KidsSuperStar.com or www.KidsSuperStar.com/contest/signupform.cfm and follow the instructions on how to register and enter.
  
  All entries must be completed by 12 am EST on Wednesday, July 1, 2009. All entries become property of MKH.



• Once registered, submit an original photo taken by you, of your child. Your photo must comply with the following photo requirements:
  

  »	The photo must be a color or black-and-white digital photo that was either taken with a digital camera or scanned into a digital format for upload. For Free mail-in entries, the photo must be 4 inch x 6 inch in size.
  »	The use of public domain images is prohibited.
  »	Photographs that include other persons will not be accepted. There should be only 1 child per photo unless they are identical twins.
  »	The photo, in its entirety, must be a single work of original material taken by the contest entrant. By entering the Contest, entrant represents, acknowledges and warrants that the submitted photograph is an original photo created solely by the entrant and that no other party has any right, title, claim or interest in the photo.
  »	Obscene, provocative, or otherwise objectionable content will not be considered, and such determination is solely at the discretion of MKH.
  »	The photograph will be judged in accordance with the Judging Criteria, as defined below.
  »	LIMIT ONE STANDARD ENTRY PER HOUSEHOLD.
  »	Odds of winning depend on the number of eligible entries received.

• No purchase is necessary to enter. To enter free be sure to follow the rules above and send one (1) 4 inch x 6 inch photo you wish to enter of your child. On a separate 3 inch x 5 inch card write your full name and your child's full name, date of birth, address, phone number and email address. Only one entry is allowed per envelope. Mail your entry to our designated data processing center:

  If you fail to comply with entry requirements and mail in requirements your entry will be void. We are not responsible for lost, delayed or damaged entries. Contest entries will not be returned and become property of MKH. Please allow 4 to 6 weeks for free mail in entries to be posted.
  
  

In addition all winners must sign an Affidavit of Eligibility. Failure to provide said affidavit within 15 days from the date of notice will result in disqualification and possible selection of an alternate winner.

Contest Prizes
The MKH 2009 SuperStar™ Search has two (2) categories:

1. Voter's Choice Award for the contestant with the most number of votes.
2. The MKH SuperStar™ Grand Prize winner selected by judges as the best overall.

MKH may award the following prizes:

• One (1) Grand Prize MKH 2009 SuperStar™ Winner will be selected from all eligible entries received. The Grand Prize Winner will receive a $1000.00 cash prize or $2000 in US Savings Bonds, a 1 year GoldStar Membership to MKH, and an 18 month Hollywood Contract! In addition the winner will be the featured face of MKH with their photo featured in either our Magazine Print ad and/or featured on www.KidsSuperStar.com¹.
• One (1) Voter's Choice Award Winner will receive a set of MKH Collector's Edition Teddy Bears and a $100 Visa or Mastercard Gift Card.
• Prizes are subject to change or substitution. In the event of a change in a prize or the unavailability of a service offered in a prize, a replacement prize of equal or greater value will be given. MKH reserves the right to provide a cash prize in exchange for a prize or service that is unavailable.
• All Prizes will be award as represented unless there are less than a total of 100 entrants. If there are less than 100 entrants, the prizes will be prorated accordingly using a percent scale then issued to the winning entrant. Example: If there are 90 entrant, the winning entrant would receive 90% of stated cash prize.
• Odds of winning depend on the number of eligible entries received.
• CASH prizes are paid in the form of a check in the name of the parent or legal guardian of the winner.

1 Grand Prize Winner has 1 year from award date to appear in person at The Kids Hollywood office in California to claim their 18 month Hollywood contract. If the Grand Prize Winner fails to appear in person at The Kids Hollywood office in California to claim their prize within the 1 year period specified, then they will forfeit their rights to the 18 month Hollywood contract and the prize offer shall expire with no additional compensation required by MKH. 

Judging Criteria
The MKH 2009 SuperStar™ Search consists of two rounds of evaluation. The Voter's Choice Award will be determined by the contestant with the most number of votes. If there is a tie, the contestant with the highest ranking will be selected as the winner.
For the Grand Prize, a panel of industry professionals will evaluate the entries based on the following criteria:

1. Photos adherence to entertainment industry standards for Commercial headshots 20%
2. Quality of photo 20% 
3. Charisma, energy and attitude displayed in photos 30%
4. Overall Appearance 30%

• MKH reserves the right to refuse any entries and photos they deem inappropriate. MKH will refuse any photos with nudity or photos with children in profane, suggestive, derogatory or dangerous positions.
• Voter's Choice Award Winners will be chosen on or about Wednesday, July 1, 2009 and notified by phone or email within 10 business days. Prizes will be released within 15 business days after the receipt of the signed affidavit of eligibility/claim form and the photo/model release.
• The MKH 2009 SuperStar™  Grand Prize winner will be chosen on or about Saturday, August 1, 2009 and notified by phone or email within 10 business days. Prizes will be released within 15 business days after the receipt claim forms.
• There will be no preference given with regard to race, color, religion, sex, handicap, familial status, or national origin.

Referral Award Program
We want to encourage everyone to spread the word about our MKH 2009 SuperStar Search™ so that every child has a chance to compete. To do this we are offering a $100 referral award to the individual that refers the child selected as the MKH 2009 SuperStar™ Grand Prize winner. Please make sure you follow our guidelines so that you will be eligible:

1. You must be 18 years old to participate in the Referral Award Program. If you are not 18, you should ask your parents or legal guardian to refer a friend for you.
2. You do not have to be related to refer someone, you can be a friend of the family, however we do not condone SPAM of any kind. If a referrer is reported for SPAM activity they will be barred from participating in the referral program and will be ineligible to receive the referral award.
3. There is no limit to the number of friends or family you can refer.
4. The mother, father and/or legal guardian of the entrant are not eligible for the referral award.
5. The entrant must indicate that YOU referred them on their online entry form at the time of entry. They must reference you by First AND Last Name and enter your email address in the box provided.
6. If you use our referral link, you must enter your First AND Last name along with your email address. This information will automatically be entered into the entry form if the person you referred signs up using the email sent to them.
7. The Referral Award of $100 will be paid by check using the name as required above within 15 business days after the MKH 2009 SuperStar™ Grand Prize winner is notified and has officially accepted their prize. If we are unable to contact the selected MKH 2009 SuperStar™  winner or they refuse to accept their prize for any reason, a new SuperStar winner will be selected which will result in the referral award beginning given to the referrer of the official winner.
8. The Referral Award will only go to the referrer of the official confirmed MKH 2009 SuperStar™  winner as determined by MKH.
9. There can be only ONE (1) referrer per entrant.
10. The person referenced at the time of entry by the entrant will be considered the sole referrer and cannot be changed.
11. If the entrant references two people at the time of entry only the first person mentioned will be considered the referrer.
12. If the Referrer is unable to be contacted after 3 attempts due to an invalid email address, an undeliverable email address or any other reason, the referral award will be forfeited with no additional compensation required by MKH. 

License
Selected entries may be published by MKH or its designee in one of MKH's brochures or on their web sites, or in any other medium, in MKH's discretion. By participating, all entrants grant an irrevocable license in the Entries to MKH, its licensees and affiliates and their designees (collectively, the "Authorized Parties") and acknowledge that any Authorized Party may utilize, edit, modify, reproduce, publish, distribute or otherwise use the Entries and all elements of such Entries, including, without limitation, the names and likenesses of any persons or locations embodied therein, in any media now or hereafter known, without the restriction, including commercially using and publishing the Entries to fullest extent possible in perpetuity, without additional consideration, notification, permission to entrant or any third party, unless prohibited by law. Additionally, by participating, each entrant grants to Authorized Parties the unrestricted right to use all statements made in connection with the Contest, and pictures or likeness of Contest participants, or choose not to do so, at its sole discretion, for no additional consideration, notification or approval except where prohibited by law.

Limitation of Liability
By participating, Entrants agree to release, discharge and hold harmless MKH and their partners, affiliates, subsidiaries, advertising agencies, agents and their employees, officers, directors, and representatives from any claims, losses, and damages arising out of their participation in this Contest or any Contest-related activities and the acceptance and use, misuse, or possession of any prize awarded hereunder.

Conditions
THIS CONTEST IS VOID WHERE PROHIBITED OR RESTRICTED BY LAW. This Contest is subject to federal, state, and local laws and regulations. Certain restrictions may apply. Entries are void if MKH determines the photograph to not be an original, or if the entries are illegible, incomplete, damaged, irregular, altered, counterfeit, produced in error or obtained through fraud or theft.

Participants also agree (a) to be bound by these Official Rules; and (b) that the decisions of the Judges are final on all matters relating to the Contest. All federal, state and local taxes, fees and surcharges on prizes are the sole responsibility of the prize winners. In the event that the selected winner(s) of any prize are/is ineligible or refuses the prize, the prize will be forfeited and MKH, in its sole discretion, may choose whether to award the prize to another winner.
